What would we do with out all the forms and paperwork? You will be able to download all the necessary forms to complete your registration with the office. You are welcome to print the forms, fill them out and bring with you or you can fill them out on-line and e-mail them back to the office. If you have any questions or need help, feel free to call and we will be delighted to help you. Thank you for choosing Surgical Eye Associates for your eye care needs. If you are a new patient to our office, or you have not be in to see us in the last 2 years, we would request that you fill out the first 4 forms. These are all in Microsoft Word format.
